No. This is a dental discount membership plan offered for care at our practice. It is not a dental insurance policy.
Membership cannot be combined with dental insurance, another dental plan, office specials, or financing programs such as CareCredit.
The periodontal plan includes four periodontal cleanings per year. Ask our team which membership is appropriate for your gum care needs and which services are included.
Membership benefits include a comprehensive new patient exam, one annual exam, and one emergency exam during the plan year. X-ray benefits have frequency limits, including a full-mouth series or panoramic X-ray once every three years. Our team can confirm your plan’s benefits; imaging is recommended according to your individual dental needs.
The membership terms describe a 12-month, nontransferable plan with no refunds or credit for unused benefits. Referred or specialty services and care covered by medical, auto, or workers’ compensation insurance are excluded. A $50 reappointment fee applies for cancellations with less than two business days’ notice. Review the current agreement before enrolling.

The plan runs for 12 months following the date of payment. Membership is nonrefundable and cannot be transferred to another person. No credit or rollover is available for unused or partially used benefits.
Membership cannot be combined with another dental plan, dental insurance, office specials, or financing programs such as CareCredit.
Payment plans are available for larger treatment plans. Ask our team which payment arrangements are compatible with membership.
Referred treatment and specialty services are excluded, as determined by our doctors. Membership cannot be used for dental costs covered by medical insurance, auto insurance, or workers’ compensation.
A $50 reappointment fee applies when an appointment is canceled with less than two business days’ notice. Contact our team as early as possible if you need to reschedule.
